40 Under Forty 20th Anniversary Celebration & Awards Program

This is the 20th annual 40 Under FORTY Awards Program recognizing 40 people under the age of 40 who have excelled in the workplace and in the community. We invite you to participate in this event by nominating an outstanding member of the community you feel is deserving of this recognition.

The event is November 16 at The SRC Arena from 4:30 - 7:30 p.m.! This year’s celebration will begin with networking and include an awards program to honor the winners.

Judging: A panel of independent judges will review each nomination submitted and assign a score to each category (Business Accomplishments, Civic Leadership, and Achievements & Awards). This is done privately, and the judges don’t share information. The scores are totaled and added together by BizEventz Director to determine the 40 honorees.

If you win: Successful nominees will be notified within 2-3 weeks after the nomination deadline. All honorees will be invited as guests of BizEventz to the 40 Under FORTY 20th Anniversary Celebration & Awards Program and recognized with an award. In addition, the Business Journal News Network will recognize those selected in advertisements promoting the event and in a Commemorative Publication that will be distributed at the event.

If you are not chosen: The nominator will be notified within 2-3 weeks after the nomination deadline. If the nominator would like BizEventz to notify the nominee, the nominator must request that BizEventz send notification. It will not be shared with the nominee unless they win.

Only hard copy applications submitted to BizEventz, Inc. or online applications will be considered. Please fill out the nomination form completely and accurately to the best of your knowledge, as this form will be used for the selection process. If the nomination form is not filled out, the candidate may score lower and then decrease their chances of making it as a winner for 2017. Please note however just submitting a resume will not replace the nomination form, but is encouraged to submit in addition to the form to help enhance the nomination.

Eligibility:
• Nominees must be 39 years old or younger as of November 16, 2017.
• Nominees are limited to companies headquartered in or with a substantial presence in the following counties: Broome, Cayuga, Chemung, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Seneca, St. Lawrence, Tioga and Tompkins.

IMPORTANT: There is NOT a Save & Return Option to complete your nomination.
Please review the items below that you will need before beginning your nomination.

Nomination Items Needed: Nominee Contact Info: name, company name, address, phone #, email.

Supporting Documents: (NOT mandatory but encouraged) to highlight and provide proof of achievements to include referral letters, resumes, certificates, awards, news articles, letters of recommendations, photos, etc. (Please note that these can be uploaded at the end of the nomination form)

Prepared Responses: You will be asked to answer a few questions about the nominee.
(Already prepared responses can be copied and pasted into text boxes within the form.) 

Written Qualifications:
The nomination and supporting documents should reflect and answer the following questions:
1.  Business accomplishments (specific achievements, special projects, business-related affiliations).
2.  Civic leadership (What impact has this person had on his/her community).
3.  Achievements, awards, and special recognitions.
4.  Family (spouse, children).
